Topical Therapy
Topical therapies are creams and lotions that can be applied to skin to assist in repigmentation in vitiligo.
These may range from topical steroids to anti inflammatory medications to newer generation biologic compounds.
If a patient’s vitiligo has progressed to most of their body, then an option to depigment and even out the colour of the whole body is an option.
These will require a consultation with your specialist and a prescription.
UVB Phototherapy
UVB phototherapy has been used with great success in the repigmentation of vitiligo patients. It works by suppressing the immune response that causes the loss of melanocytes, as well as activates functioning melanocytes to produce more pigment.
Regular treatment, usually 3 times per week for 4 months to begin with, is required. If there is success of repigmentation within this period, then treatment continues until repigmentation is satisfactory and can continue for as long as required.
Systemic Therapy
In rapidly progressing vitiligo, your dermatologist may prescribe oral steroids or immunosuppressants to halt the progression of the disease. They are often used in combination with topicals and phototherapy.
Laser & ReCell
In cases of stable, resistant vitiligo, particular smaller, cosmetically sensitive areas of concern, fractional laser or ReCell melanocyte grafting may be considered.
Fractional lasers tunnel microscopic grids into the skin, at which point topical medications are applied (laser-assisted drug delivery), potentially causing a greater improvement in repigmentation.
ReCell is a technique known as “autologous melanocyte transplant” whereby skin from a pigmented area of the body is harvested, processed and transplanted into an area of vitiligo, with the hope of causing repigmentation.
Dr Manoharan has extensive experience in the use of both ablative and fractional laser in combination with ReCell and has achieved some excellent outcomes in patients with stable vitiligo.
